 “Planning for Your Future – If Not Now, When?” 

Some conversations are hard to have but are, nonetheless,  very important.  This presentation will help you ponder your healthcare future and enable you to empower loved ones with information they need to know about your wishes. Then, and only then, will they be able to speak on your behalf if you can no longer speak for yourself.

We are about to celebrate Rosh Hashanah 5784, and IYH it should be a year of good health, happiness, and peace. May we all be sealed in the Book of Life. Yet, the fragility of life encourages us to consider how to do better for ourselves and our families by planning what we can plan. So, let’s take steps to get our house in order by becoming familiar with an easy-to-understand resource called the Five Wishes.

Presented by Toby Weiss, MS, OD

Toby Weiss holds an MS in HRM and OD, and a certificate in Biomedical Ethics. She is AVP of Cultural Diversity at MJHS Hospice and improves equitable access to care while embracing the needs of all who are seriously ill.  She educates on the importance of advanced care planning and earlier access to healthcare resources in alignment with a person’s lived experiences.  With a focus on Person-Centered, Trauma-Informed care Toby has done in-depth work to support the needs of populations with a history of trauma. She was selected to serve on the national CMS Health Equity Expert panel that developed Health Equity measures for Hospice and Homecare Quality Reporting.  Toby established the MJHS “Halachic Pathway” for Orthodox Jewish patients and has become a replicable model for other cultural and faith-based communities. Her team created and rolled out a digital toolkit aimed at caring for Holocaust Survivors at the end of life, which won the AJAS National Jewish Programming award in April 2022.
