Creative/Graphics Request Form for OU Depts.

For all graphics/creative requests, please answer the below questions as thoroughly as possible. A new Request Form is required for each individual request.

Is your request something that was already being worked on/discussed in Zendesk (creative@ou.org) If yes, please provide a link to the ticket.
Do you have a specific vision for this piece? (e.g. humorous, include image or photo, inspirational, etc.)
