{"id":62426,"date":"2019-11-06T12:55:13","date_gmt":"2019-11-06T17:55:13","guid":{"rendered":"https:\/\/www.ou.org\/life\/?p=62426"},"modified":"2019-11-06T12:55:13","modified_gmt":"2019-11-06T17:55:13","slug":"the-overuse-of-antibiotics","status":"publish","type":"post","link":"https:\/\/www.ou.org\/life\/inspiration\/the-overuse-of-antibiotics\/","title":{"rendered":"The Overuse of Antibiotics"},"content":{"rendered":"<p class=\"p1\"><span class=\"s1\">According to the Centers for Disease Control and Prevention (CDC), at least 30% of antibiotics in this country are overprescribed and are unnecessary.<span class=\"Apple-converted-space\">\u00a0 <\/span>Why?\u00a0<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Antibiotics are wonderfully important, especially when necessary.<span class=\"Apple-converted-space\">\u00a0 <\/span>The world should express gratitude to Sir Alexander Flemming (1881-1955) for inventing the world\u2019s first antibiotic substance benzylpenicillin (Penicillin G) from the mold called <i>Pencillium notatum<\/i> in 1928.<span class=\"Apple-converted-space\">\u00a0 <\/span>Sir Flemming was awarded the Nobel Prize in 1945, which he shared with two colleagues.\u00a0<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Another thank you is owed to Dr. Selman Waksman (1888-1973), a Ukrainian Jew who emigrated to the United States in 1910 and received his Ph.D. in biochemistry in 1918 from the University of California.<span class=\"Apple-converted-space\">\u00a0 <\/span>Dr. Waksman discovered streptomycim, the first antibiotic that was effective against tuberculosis.<span class=\"Apple-converted-space\">\u00a0 <\/span>In 1952, he was awarded the Nobel Prize in physiology.\u00a0<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">These and many other microbiologists and other health care professionals revolutionized health care for the better through their research and medicine.\u00a0<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Imagine a world, G-d forbid, without antibiotics.<span class=\"Apple-converted-space\">\u00a0 <\/span>This would include, for example, no treatment for strep throat.<span class=\"Apple-converted-space\">\u00a0 <\/span>Strep throat, which is more common in children and teenagers (especially ages 5-15) than adults, is an infection in the throat and tonsils caused by bacteria called group A <i>Streptococcus <\/i>(Group A strep).<span class=\"Apple-converted-space\">\u00a0 <\/span>Group A strep live in the nose and throat and can spread to other people through coughing and sneezing.<span class=\"Apple-converted-space\">\u00a0 <\/span>Strep throat does not always present with sick symptoms.<span class=\"Apple-converted-space\">\u00a0 <\/span>Left untreated, it can cause kidney inflammation or rheumatic fever which can weaken the heart.\u00a0<\/span><\/p>\n<p class=\"p3\"><span class=\"s2\">And yet, not every sore throat is a strep throat.<span class=\"Apple-converted-space\">\u00a0 <\/span>Sometimes, a sore throat is\u2026just a sore throat.<span class=\"Apple-converted-space\">\u00a0 <\/span>There is <\/span><span class=\"s3\">pain, a scratchiness or irritation of the throat that often worsens when you swallow. The most common cause of a sore throat (pharyngitis) is a viral infection, such as a cold or the flu. A sore throat caused by a virus resolves on its own.<span class=\"Apple-converted-space\">\u00a0 <\/span>You read this correctly \u2013 It resolves on its own.<span class=\"Apple-converted-space\">\u00a0 <\/span>This means there is no medicine to make it go away, although Mommy\u2019s chicken soup helps.<span class=\"Apple-converted-space\">\u00a0 <\/span>For real.<span class=\"Apple-converted-space\">\u00a0 <\/span>In addition to the ingredient known as \u201clove\u201d and never to be underestimated, the liquid and vegetables (which are also high in liquids) causes one to urinate.<span class=\"Apple-converted-space\">\u00a0 <\/span>Staying properly hydrated and flushing out the system is important every day, and especially when feeling unwell.\u00a0<\/span><\/p>\n<p class=\"p5\"><span class=\"s1\">Let\u2019s now talk about another malady for which people believe antibiotics are the cure all &#8212; ear infections.<span class=\"Apple-converted-space\">\u00a0 <\/span>Children are more prone than adults to ear infections.<span class=\"Apple-converted-space\">\u00a0 <\/span>Unfortunately, they can be uncomfortable and even painful.<span class=\"Apple-converted-space\">\u00a0 <\/span>Yet not all ear infections require antibiotics.<span class=\"Apple-converted-space\">\u00a0 <\/span>Seriously.<span class=\"Apple-converted-space\">\u00a0 <\/span>It is important that a physician or experienced health care provider look in the ears to diagnose correctly whether there is an infection and, if there is an infection, whether it requires antibiotics.\u00a0<\/span><\/p>\n<p class=\"p5\"><span class=\"s1\">Believe your doctor when he tells you that antibiotics are not at that moment warranted and you should wait it out.<span class=\"Apple-converted-space\">\u00a0 <\/span>G-d has miraculously created the human body so it can often heal itself.<span class=\"Apple-converted-space\">\u00a0 <\/span>Introducing antibiotics when not needed can disturb the cosmos order of the human body and not allow the body to develop its own antibodies which is a necessary part of life.<span class=\"Apple-converted-space\">\u00a0 <\/span>Antibodies are produced by white cells that have a memory to the foreign antigen (bacteria or virus).<span class=\"Apple-converted-space\">\u00a0 <\/span>These antibodies attack the bacteria.\u00a0<\/span><\/p>\n<p class=\"p5\"><span class=\"s1\">Experienced parents are familiar with this scenario.<span class=\"Apple-converted-space\">\u00a0 <\/span>Nonetheless, some will beg for antibiotics even when both the rapid strep test and the overnight throat culture are negative or the ear infection is one which should be waited out (with the help of Tylenol and Motrin).<span class=\"Apple-converted-space\">\u00a0 <\/span>Your child may be getting the strep throat.<span class=\"Apple-converted-space\">\u00a0 <\/span>Notice the verb tense \u2013 <i>getting<\/i>.<span class=\"Apple-converted-space\">\u00a0 <\/span>Sometimes, it takes time to emerge and I respect a parent, especially a mother who is asking for the antibiotics at this point, because she will know her child best.<span class=\"Apple-converted-space\">\u00a0 <\/span>But to give antibiotics without proof of infection is not good medicine.\u00a0<\/span><\/p>\n<p class=\"p5\"><span class=\"s1\">Plenty of people go to urgent care or PAs in their homes for throat cultures or to have their children\u2019s ears looked at.<span class=\"Apple-converted-space\">\u00a0 <\/span>Sometimes, I even get a report (which is legally mandated, by the way).<span class=\"Apple-converted-space\">\u00a0 <\/span>So often, the report will state that the tests were negative.<span class=\"Apple-converted-space\">\u00a0 <\/span>Scroll down a drop further and it will state that Amoxicillin or something similar was prescribed. <\/span><\/p>\n<p class=\"p5\"><span class=\"s1\">Huh?<span class=\"Apple-converted-space\">\u00a0 <\/span>If a test is negative, then there should be no antibiotics.<span class=\"Apple-converted-space\">\u00a0 <\/span>Period.\u00a0<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">You, my dear reader, might be scratching your head right now, wondering why physicians are against gratuitous prescriptions.<span class=\"Apple-converted-space\">\u00a0 <\/span>The answer is simple \u2013 because unwarranted antibiotics can cause more harm than good.\u00a0<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">Let\u2019s say you become sick with a gastroenteritis, a stomach flu.<span class=\"Apple-converted-space\">\u00a0 <\/span>Antibiotics are not going to get you better.<span class=\"Apple-converted-space\">\u00a0 <\/span>Yet if you take them, you will make your body and germ bacteria (which abound in each of us) drug-resistant to antibiotics.<span class=\"Apple-converted-space\">\u00a0 <\/span>Antibiotic resistance is a problem in today\u2019s world.<span class=\"Apple-converted-space\">\u00a0 <\/span>When you really need antibiotics, they may not work to eliminate the harmful bacteria.\u00a0<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">If the antibiotics do not work when needed you could be looking at more aggressive intervention.<span class=\"Apple-converted-space\">\u00a0 <\/span>This will take a toll on the body.<span class=\"Apple-converted-space\">\u00a0 <\/span>There may be more expensive medications, increased health care bills and hospital stays.<span class=\"Apple-converted-space\">\u00a0 <\/span>There could be multiple office visits to your primary care provider and\/or specialists to find the correct antibiotics.<span class=\"Apple-converted-space\">\u00a0 <\/span>Because of the overuse of antibiotics, the field of infectious diseases is challenged to finding appropriate antibiotics.\u00a0<\/span><\/p>\n<p class=\"p1\"><span class=\"s1\">There are medicines today for bacterial infections that not so long ago prevented the spread of disease and even reduced serious complications.<span class=\"Apple-converted-space\">\u00a0 <\/span>Unfortunately, overprescribing them has led to resistant bacteria to pneumococcal infections like pneumonia, meningitis, MRSA or <\/span><span class=\"s4\">Methicillin-resistant Staphylococcus aureus, and tuberculosis, to cite a few examples.\u00a0<\/span><\/p>\n<p class=\"p7\"><span class=\"s1\">It is becomingly increasingly less uncommon that when there has been an over usage or misusage of antibiotics, there are abnormal bacteria that proliferate in the gastrointestinal tract.<span class=\"Apple-converted-space\">\u00a0 <\/span>The \u201cgood\u201d bacteria are killed by antibiotics.<span class=\"Apple-converted-space\">\u00a0 <\/span>C-difficile, which not so long ago was a sickness found in nursing homes and hospital acquired infections, is becoming prevalent in the general population.\u00a0<\/span><\/p>\n<p class=\"p7\"><span class=\"s1\">The havoc that it wreaks on a person\u2019s bowels and their frequent trips to the bathroom are far from pleasant.<span class=\"Apple-converted-space\">\u00a0 <\/span>The antibiotics for this are difficult to administer to a child.<span class=\"Apple-converted-space\">\u00a0 <\/span>Fecal transplants, which are needed to reset healthy gut flora, are becoming growing in number.\u00a0<\/span><\/p>\n<p class=\"p7\"><span class=\"s1\">Further, studies from Israel show that the earlier the introduction of antibiotics to an infant or child, the greater the chance of their becoming obese when older.<span class=\"Apple-converted-space\">\u00a0 <\/span>Why?<span class=\"Apple-converted-space\">\u00a0 <\/span>Because overusing and\/or misusing antibiotics changes the gut flora.\u00a0<\/span><\/p>\n<p class=\"p8\"><span class=\"s5\">Then there are superbugs.<span class=\"Apple-converted-space\">\u00a0 <\/span>You read it correctly.<span class=\"Apple-converted-space\">\u00a0 <\/span><\/span><span class=\"s6\">Carbapenem-resistant\u00a0<i>Enterobacteriaceae<\/i>\u00a0(CRE) are part of a group of germs that live in the intestines of some people. They&#8217;re related to\u00a0<i>E. coli<\/i>, which are normal to have\u00a0in your intestine and stool.<span class=\"Apple-converted-space\">\u00a0 <\/span>The difference is these germs mutate and become resistant to antibiotics. Originally from South Africa, he started his current practice, Total Family Care of the Five Towns and Far Rockaway, PC in 1987. Dr. Lightman is a board-certified pediatrician and fellow of the American Academy of Pediatrics (FAAP). Over the years, Dr. Lightman has served in a number of leadership roles: director of the adult allergy clinic at Louis Lasky Center, director of pediatric allergy and asthma at Queens General Hospital, and co-director of pediatric and adult outpatient allergy at St. John\u2019s Episcopal Hospital. Dr. Lightman is also a clinical assistant professor of pediatrics at Hofstra Northwell School of Medicine. In addition, he is actively involved in teaching pediatric and family nurse practitioners through Columbia University, Pace University, Lehmann College, and Molloy College, as well as mentoring physician assistants through Touro College. As the Medical Director and a practicing physician, Dr. Lightman has an accessible and friendly manner with parents and children alike. He holds himself and the whole practice to the highest standards of medical excellence, whether it be a simple camp physical or a child with serious symptoms.
