So\, let’s take steps to get our house in order by becoming familiar with an easy-to-understand resource called the Five Wishes. \nPresented by Toby Weiss\, MS\, OD \nToby Weiss holds an MS in HRM and OD\, and a certificate in Biomedical Ethics. She is AVP of Cultural Diversity at MJHS Hospice and improves equitable access to care while embracing the needs of all who are seriously ill.  She educates on the importance of advanced care planning and earlier access to healthcare resources in alignment with a person’s lived experiences.  With a focus on Person-Centered\, Trauma-Informed care Toby has done in-depth work to support the needs of populations with a history of trauma. She was selected to serve on the national CMS Health Equity Expert panel that developed Health Equity measures for Hospice and Homecare Quality Reporting.  Toby established the MJHS “Halachic Pathway” for Orthodox Jewish patients and has become a replicable model for other cultural and faith-based communities.
